We ask that you bring your insurance card to each visit and be prepared to pay your co-payment at the time of your visit. Please remember that each child requires a separate co-payment. Because of the continual complexity of insurance companies we suggest that you confirm your policy coverage details with your insurance company.

You can call for an appointment during normal business hours, Monday- Friday, 8:00 am to 12:00 noon and 1:00 pm to 5:00 pm. The office number is 719-633-3850; fax 719-227-0840. If a child will be accompanied by anyone other than a parent or legal guardian, a form giving permission is required; see the Patient Forms section of this web site.
