How do I place someone in your facility? A. Please call our facility to schedule a tour and complete an application for admission. We will need to obtain medical information from the prospective resident's physician or hospital discharge planner. Does the facility accept Medicare and/or Medicaid are they different?

Capstone is a certified residential skilled nursing facility, providing short-term rehabilitation and long-term care to individuals with health conditions that require higher levels of medical and nursing services. Under the supervision of Capstone's medical director, a team of physicians cares for each resident.
Social Services

Capstone has social workers on staff to help the resident and their family members adjust to placement. Working with the psychosocial needs that may arise during the course of the individual's stay at the facility, the social worker is not only a source of emotional support, he or she also assists with any necessary adjustments to facility living, mood, financial, and individualized concerns or family requests.
