If you are unable to print out this paperwork, please arrive 15 minutes prior to your appointment at our Brookfield chiropractic office. It is also important to bring in a form of ID as well as any insurance cards that you would like to have a complementary benefits check run on. Next, you will sit-down for a one-on-one consultation with one of our chiropractic doctors.

The Palmer Package is a compilation of techniques taught at Palmer College of Chiropractic in Davenport, Iowa- The birthplace of Chiropractic, and Dr. Jon and Dr. Kylee's Alma Mater. Palmer Package Adjusting can be described as "Traditional Chiropractic Adjustments" or "Manual Adjusting". This technique is the most used of all chiropractic techniques and is the one probably most familiar to patients.
