Dr. Nicholas J. Roumpakis will be performing your CDL/DOT physical. Dr. Roumpakis is one of a few licensed chiropractic physicians in the country that is a Certified Medical Examiner being listed in the National Registry of Certified Medical Examiners. The National Registry is a division of the United States Department of Transportation and the Federal Motor Carrier Safety Administration.
The (FMCSA) requires all medical examiners (MEs) who wish to perform physical examinations for interstate commercial motor vehicle (CMV) drivers to be trained and certified in FMCSA physical qualification standards. Medical examiners who have completed the training and successfully passed the test are included in an online directory on the National Registry of Certified Medical Examiners website.

John attended Purdue University Northwest in Hammond, Indiana, Indiana University Northwest in Gary, Indiana, and received a Bachelor of Science degree in Health Sciences from Chicago State University in Chicago, Illinois. He continued his education at Valparaiso University, Valparaiso, Indiana, where he received his Bachelor of Science degree in Nursing.

The Federal Motor Carrier Safety Regulations defines a Medical Examiner as a person who is licensed, certified, and/or registered in accordance with applicable state laws and regulations to perform physical examinations. The term includes but is not limited to doctors of medicine, doctors of osteopathy, doctors of chiropractic, physician assistants, nurse practitioners, and advanced practice nurses.

This procedure will make your office appointment time more efficient. If you cannot download these forms, or cannot complete them prior to your visit, they will be provided at our office. Please arrive 30 minutes prior to your appointment time to allow ample time for completion of forms. If you have been diagnosed with a diabetic condition-please bring a list of medications and the name of the prescribing doctor.
