Eden Pathways operates a Comprehensive Home Health Care Agency in the Twin Cities and Metro areas. We provide primarily community-based services that utilize the skills and services of a Registered Nurse (RN), Licensed Practical Nurse (LPN), Personal Care Assistants (PCA), Home Health Aides (HHA), Homemakers, and Companions.
We also provide Private Duty Nursing services. We work together as a team in an effort to restore function, improve mobility, and to enhance our clients overall physical, emotional, mental and cognitive health.

From their assessment they determine what additional services may be needed (for example physical therapy, or the need to apply for medical rides). Our aim is to ensure than an optimal comprehensive care is delivered. Our LPN's are the team members that implement the care plan and provide day to day/hands on nursing care to our patients in their homes.

PRIMARY RESPONSIBILITY: The Licensed Practical Nurse is responsible providing clinical nursing services to home care clients. Performs focused nursing assessment of clients to determine appropriate interventions to maintain or improve the client's health status. Provides interventions for clients with episodes of potentially life-threatening instability.
Personal Care Assistant PCA

PRIMARY RESPONSIBILITY: The Personal Care Assistant is responsible for providing direct care to home care clients. Perform additional delegated tasks, including medication administration, upon completion of instruction and return demonstration. Maintains absolute confidentiality of all information pertaining to clients, families, and employees.

Eden Pathways has launched its "Pathways to Excellence" Scholarship for 2021. This scholarship offers funding to eligible caregivers who are interested in advanced education in various topics/disciplines, and also who are interested in pursuing CNA certification, LPN, RN career paths.
