I have a great passion for smooth transitions for patients as they move between facilities and acted on that passion by chairing the community action group in Washington County called Community Care Transitions Team.

The patient must be essentially homebound, meaning trips away from the home must be infrequent or of short duration. Medicare does allow trips outside of the home for religious services and medical treatment. Care must be intermittent (part time), medically necessary, and ordered/approved by your physician.

Each year, more than 20,000 Oklahomans receive services through the AdVantage Program. This program is provided by the Home and Community-Based Services division of the Oklahoma State Department of Health. These Medicaid services help people stay in their home instead of going into a nursing home. The program assists frail elders and adults who have physical disabilities.
