Over the coming year, our practice will be transitioning to a patient centered medical home (PCMH) practice. Becoming a PCMH practice involves accreditation from the National Committee for Quality Assurance (NCQA), a non-profit organization that works to improve health care quality through the administration of evidence-based standards, measures, programs, and accreditation.

We are committed to providing you with the best possible care and treatment. If you have medical insurance, we are anxious to help you receive the maximum allowable benefits. In order to achieve this goal, we need your assistance and your understanding of our payment policy. A $10.00 fee will be assessed if a required co-payment is not paid at the time of service and the patient account must be billed.
