Upon entering our office, our front desk associate will welcome you and have you sign in. If you did not get a chance to fill out the paperwork online, you are welcome to do that in our waiting area. However, it will save you time if you are able to submit the paperwork before your first visit. It is wise to bring any diagnostic images, reports, or other healthcare related files to your first visit if they are recent within the year.
