Beloved Community Family Wellness Center provides health care for all persons, regardless of their financial status. The sliding fee discount scale payment plan will be developed based on your family size and income. By offering this special service, you can always be assured to receive health care for you and your family.
Patient Portal

If you do not have an e-mail address, you can still have access to your health information by entering your token number and birthdate at the enrollment screen. You will be asked for your e-mail address, but it is not required if you do not have one at this time. You have the opportunity to communicate with Beloved Community Family Wellness Center if you do not have an e-mail address.
Dental Clinic

Our dental clinics are conveniently located at both our Robbins and Halsted (Chicago) locations. We provided the full spectrum of dental services. Thank you to all of our staff, patients, members, affiliates, partners, families and friends for joining us. We now offer accessible, affordable, and quality dental care in Englewood.
