Located in Glendora at the foothills of the San Gabriel Valley our 118 - bed nursing center features 84 SNF beds and 34 subacute beds. Sub-Acute

Sub acute care is defined as comprehensive inpatient tracheostomy and/or ventilator care designed for someone who has an acute illness, injury or exacerbation of a disease process. It is goal-oriented treatment given immediately after, or instead of, acute hospitalization to treat complex medical conditions or to administer technically complex treatments.

Our experienced rehabilitation team is committed to providing outstanding results and excellent quality of care. Residents are empowered through individualized programs to reach goals of increased independence and ability. Resident families are also encouraged to participate in therapy sessions, caregiver education, and training.
