We know your time is valuable, so we want to make your first visit with us as easy as possible. Prior to your first session, please take a few minutes to complete your new client forms. You will be asked to create a free online account. If you are a new client attending your first appointment at Cornerstone, please plan to arrive 10 minutes early.
