After requesting an appointment, please check your email for a message confirming your appointment. New patients will need to register on our Patient Portal and submit in-take forms before their appointment. Forms can be submitted here!. If a patient has missed their appointment and not rescheduled, a no show fee of $25 will be charged at the end of the work day.
