Dr. Perales is board certified Family Doctor and Clinical Assistant Professor of Family, Community and Preventive Medicine at the University of Arizona College of Medicine - Phoenix. Prior to opening Live Well Family Medicine in Chandler, AZ she worked as a primary care physician and urgent care physician for the Maricopa Integrated Health System in Phoenix, AZ and for Kaiser Permanente in northern California.

Dr. Zuzak answered all our questions promptly and professionally. My whole family of four went for the visit and she took the time to listen to all of our concerns without us feeling rushed.
