Personal Care

At Home Care provides In Home Services through Medicaid approved programs. Our goals are to assist eligible individuals to remain in their own homes while receiving home and community-based aide services, aid parents and families in caring for their disabled and/or special needs children while assisting with activities of daily living.
Private Duty Care

At Home Care provides Private Duty care which helps clients of all ages to safely remain while living in their homes or assisted living facilities. Services are available 24 hours a day, 7 days a week, for as long as the client and family agree to participate with the plan of care. Consumer Directed Services

At Home Care provides Consumer Directed Services (CDS). CDS is a Medicaid based program which provides services to individuals with disabilities similar to in home care, enabling them to live independently. Consumers in the CDS program are trained to hire their own attendants to assist them with daily tasks.

Deserving seniors should not fail to take advantage of the benefit because of an inability to pay for care during VA processing period. Our assistance program alleviates this frustration and creates opportunities that would be missed. Veteran served 90 days or more of active duty with at least one day during a wartime period.
Nursing

At Home Care clinical staff coordinates care in the case of: multiple chronic illnesses or multiple hospitalizations.
