Most insurance companies cover self-referral to physical therapy. We will let you know BEFORE your first visit if your insurance has any special referral requirements. Before your first appointment, our administrative staff will contact your insurance company, check your benefits and then explain them to you.

Call one of our offices for more information and if you have any questions about self-referral, our services or your insurance coverage. Our goal is to provide you and your family the same service we expect our family would receive at a physical therapy office. Christopher DiPasquale established the physical therapy practice with the Hebron office in 1989 and the Colchester office in 1990.

Self-refer
Report
Doctor's will often send you for x-rays, try medications and schedule return visits. If you don't get better you will be sent for an MRI or to a specialist who will order an MRI and try different medication. Somewhere along the way or if you don't get better you will be sent to physical therapy and get better.

We are looking for a fulltime Physical Therapist to join our progressive orthopedic physical therapy practice stressing direct access. We have one opening in our Hebron office. We also have part-time and per diem opportunities available in both offices. Only apply if your goal is to provide the same treatment you would expect to receive or would provide your family.
Insurance

Performance Physical Therapy accepts Medicare, most major insurance plans and private pay clients. Performance Physical Therapy administrative staff will check your benefits and explain your situation before your first appointment. Most insurance companies provide the same coverage whether you self-refer or obtain a referral from your primary care provider.

We specialize in helping people avoid medications, injections, and surgery they may not need. We also specialize in the treatment and rehabilitation of people that have to have orthopedic surgery. This includes all types of shoulder, arm, and hand surgery. Also all types hip, knee, ankle, and foot surgery.
