Healthy Life Choices' vision is to provide therapy, intensive in-home case management, and quality mental health services to individuals who are living with a mental illness, and/or providing support to family members who are living with a mental illness. Our mission is to remove barriers of access to quality mental health services within the local community.

Medical Records Request

Medical records hold important information about your health history, diagnoses, conditions, and treatment plans. This includes medications you may have received while at one of our facilities. Please keep in mind that medical records are confidential. To protect this information, your written authorization is required before a copy can be released.

HLCGA Services offers statewide supervision to help you to set goals, maintain healthy boundaries, interview preparation, reduce anxiety, and increase confidence.
We support social workers with case review, new or unfamiliar cases when working with clients with diverse backgrounds when dealing with complicated situations, which can raise potential risks of dual relationships, conflict of interest, telehealth, ethical issues (including mandated reporting), diagnostic skills, treatment planning, and interventions.
