Dr. Okezie's staff is always friendly and ready to greet you and your little one warmly, come in and see us today! Had a bad experience with our previous pediatrician. Then I found Dr. OKEIZE office. They are just the best; every member of this staff is the sweetest. When I call to set an appointment the office staff know the kids and asks me how they are doing.
Dr. Jessica is absolutely amazing. My son is always quiet around adult but he will interact with her and loves to see her. Dr. Okezie is the sweetest and very attentive to all the kids. Having a newborn is an exciting time in your life. You may wonder how often your baby will need to have a wellness visit. As each child is unique, their check up schedule may vary but don't worry, we will guide you every step of the way! And if you have any questions or concerns, we are here for you.
Well-Child Visit

Childhood is a time of rapid growth and change. Your child will have more pediatric well-child visits when they are younger and developing most quickly. Each visit includes a complete physical examination. During this exam, we will check your child's growth and development in order to spot and prevent problems.

This is a permission form for adults (18yrs and older) other than the parents or legal guardians to bring the child(ren) to the office for medical care, and to give consent for medical treatment.

Kindergarten Physical Form
Report
To protect the health of children, California law requires a health examination on school entry. Please have this report filled out by a health examiner and return it to the school. The school will keep and maintain it as confidential information. If you do not want your child to have a health check-up, you may sign the waiver form (PM 171 B) found at your child's school.
