An Innovative Care serves residents who are ambulatory, semi-ambulatory or non-ambulatory and who may also be of advanced age, physically disabled, terminally ill, or diagnosed with Alzheimer's or a related dementia. Each resident unit is furnished with a bed, dresser, closet/wardrobe, and has plenty of space for residents to personalize their room with belongings.

We are partnered with several Managed Care Organizations (MCO) to accept Family Care, and Partnership programs. Family Care is a Medicaid long-term care program for frail elders, and adults with physical, developmental, or intellectual disabilities. Our MCO partnerships include: Community Care, iCare, and My Choice Wisconsin.
