Clark County Rehabilitation & Living Center has provided care for the elderly, mentally ill, and developmentally disabled in our rural Wisconsin community since 1922.

Clark County Rehabilitation & Living Center has historically provided services for behaviorally challenged individuals to counties throughout the state since 1922. Our services have allowed other counties to offer greater options along with the continuum of care for their residents.
A collaboration of several counties throughout Wisconsin, the Commission is contracted with Clark County Rehabilitation & Living Center to maintain and expand special services.The commission was created by an intergovernmental agreement under Wis. Stats 66.0301 consisting of member counties that hold the license for operating Clark County Rehabilitation & Living Center's nursing home and facility for the developmentally disabled (ICF-ID).

Admissions

Rely on our team of experienced professionals to assist you during this transition and make your admission as smooth as possible. Most of our admissions are referrals from hospitals, clinics, other nursing homes, and assisted living facilities, but you can also contact us directly. Prior to admission, we complete an assessment with your physician so we can tailor our services to your needs.
Rehabilitative Care

Treatments can be up to seven days a week with one or two visits per day. Each treatment lasts 30-60 minutes (frequency and length of sessions vary depending upon your needs). You will meet with a team consisting of your nurse, social worker, therapist, dietian, and activity coordinator to determine your therapy goals and the anticipated length of stay.

I was there April 15th 2019 to September 5 2019.
I was in room 101 first person to stay there on South side. Nice people food over cook ,frozen activity to do West side is very awsome people made craft terment. TV remote work no numbers on TV remote. Pool table, birds, get your hair cut , canteen they handle your money so won't get stolen! Library rehab knows what they are doing! I go back there if I need to!
There is 11 wells because they live in country State of winconsin owe it and adult day care on same property.i had to teach employees how to use TV remote and witch channel games at on.
