In order to optimize your custom patient experience, we have enclosed a series of documents to be completed prior to you first face to face visit with one of our highly trained physicians. In the state of Ohio, a patient must qualify with one of more of the qualifying conditions in order to received a physician recommendation for treatment with medical cannabis.

Please refer to the enclosed list. Our services will include general family medicine, general gynecology, acupuncture, and medical cannabis as well as apitherapy, natural nutrition and essential oils. We will tailor your services to your needs and will be happy to work with your HSA, HRA, and/or FSA.
