Centric Physicians is a Medical practice providing healthcare to patients in assisted living and independent living communities, skilled nursing and long term care facilities, acute rehab facilities, long term acute-care hospitals, and acute care hospitals. We are a full spectrum practice focusing primarily on the care of patients age 45 and above in our post-acute settings.
Centric Physicians Outpatient Clinics were developed to provide care to all adults ages 18 and above while also closing gaps in geriatric care. Our convenient locations in Stone Oak San Antonio and Boerne provide assistance for patients prior to assisted living move in. We can provide a primary care visit that will be able to authorize a resident to safely and comfortably move into their new home.
