Print out the forms and bring them in filled out on your first. Upon completing your health history forms, one of our amazing chiropractic assistants will take you on a tour of our state-of-the-art corrective care center so you can better get to know us. Your time is important to us, so we strive to get every new patient back to see the doctor as quickly as possible to begin the discovery process.

Your time is valuable, so we make every effort to create an efficient experience with every visit. A touch screen check-in streamlines your visit by automatically letting the doctor know you've arrived. Choose a spot in the "hot seats" and you'll be called promptly for your adjustment. Our paperless system means your x-rays and progress details are presented to the doctor at each treatment.
