Veterans Program - VA Contract Nursing Facility

Silver Springs Care Center's contract with the Veterans Administration allows us to serve our veterans, especially those utilizing service-connected benefits, for a short term rehabilitation or long term care stay including with behavioral health and substance use treatment programs.
We hold a weekly Veterans Group for residents who have served in the Armed Forces and the facility is working within the community to increase socialization and quality of life for our veterans.Case Managers from the Veteran's Administration in West Haven, CT visit and follow our residents to assist with appropriate services and day programming.

Greater Hartford Memory Care Center

Silver Springs is a Greater Hartford Memory Care Center. At GHMCC, our dementia care team guides and supports you and your family members as they transition to our centers. Upon admission to our program, residents' specific needs and lifestyle patterns are identified and addressed through the assessment and individual care planning process.
Behavioral Health Program

Silver Springs Care Center's Behavioral Program is a well developed, thoughtful, professional and proven approach to caring for residents with behavioral health needs. The program consists of teaching, coaching and improving the lives of the residents through education, patience and guidance. Safety and security are carefully considered and assured while respecting the dignity and rights of our residents.
Respite Care Services

Respite Care provides caregivers a temporary break, or time of relief, from their care giving duties. It is ideal for family caregivers in need of a break for vacation or other travel or for loved ones needing assistance in recovering from a health circumstance or life transition. Respite Care also serves to provide a break from the constant physical and emotional stress of caring for a person with 24/7 needs.
Social Services - Care Planning - Discharge Planning

At Silver Springs Care Center, our social services department works with residents, families and community agencies to facilitate a positive adjustment to the life at the facility and prepare for services at the next site of care, ideally re-entry into the community and living at home. Upon discharge follow up phone calls are made for 90 days to ensure successful community re-entry.
