It includes descriptions of our office location, including a map and directions, hours, appointment scheduling, insurance acceptance and billing policies. We strive to make productive use of our patients time and do not double book our schedule, meaning that we strive to run on time. For this reason we have a 24 hour cancellation policy.
New Patients

If you are a New patient who is interested in scheduling with our practice please call our office at (208) 287 - 5525. Whether or not you are a medical or cosmetic patient we need you to fill out online patient paperwork which we will grant you access to over the phone, once paperwork is completed we can place you in the schedule.
