Family Health Services is a non-profit, Federally Qualified Health Center (FQHC) providing quality, affordable health care to all. Our doctors and nurse practitioners strive to improve the health of our community by providing primary care services to individuals and families of all ages and backgrounds.
We have the unique ability to offer medical services to people from all walks of life to afford high quality health care services. We accept Medicaid, Medicare, and plans from most insurance carriers. We also use a sliding scale in our practice for patients who do not have insurance or whose coverage failed to take care of the bills.

Meet Our Providers & Staff

Ms. Bertke graduated from The University of Toledo with a Bachelor of Science in Nursing. She then continued her education at Walden University, where she successfully earned a Master of Science in Nursing and her certification as a Family Nurse Practitioner. Ms. Bertke provides care for patients of all ages, and has been a Certified Nurse Practitioner at Family Health Services since 2015.

You first need to fill out our New Patient Packet. This can be downloaded from our Forms page or can be acquired at the FHSEC office you would like to attend. We offer a sliding fee scale at Family Health Services. The application is available here. All self-pay patients are requested to make a $12 payment at the time of their office visit.

Family Health Services is now offering our patients easy and private access to your medical information online, so you can view your personal health record whenever and wherever you have access to the Internet. How do I access the Patient Portal? To gain access to our secure server on the Patient Portal and become web-enabled, simply sign up by providing us with a personal (non-work) e-mail address.
