Mint PT offers personalized physical therapy services and programs to support both orthopedic and neurological diagnoses. Our staff has specialized training to deal with a wide array of injuries, conditions, and treatment scenarios. Offering a full range of Neurological & Orthopaedic Therapy services.
Your First Visit
Report
Prior to your first visit, we will email you paperwork to make your first visit more efficient. If you are unable to print the paperwork, we ask that you arrive 15 minutes early to fill it out in the office. You will complete a medical history form, sign standard HIPPA forms, a financial agreement form, and usually a standardized outcome measurement concerning your injury.
