ALL OF THE FORMS will be needed (with the exception of the 'Auto Accident' if you are not being seen due to an accident). Comfortable clothing that is easy to move in and allows us to get to your area of complaint. There are few absolutes in healthcare. We will evaluate the information gathered to develop a working diagnosis.
