We understand that the need for post-acute care is often unplanned, and we offer admissions seven days a week for the smoothest possible transition between care settings. A personal care assessment determines the appropriate level of care and establishes preliminary goals.
Our dedicated core team includes nurses and nursing assistants, full time nurse practitioner, rehabilitation therapists, activities staff, a dietitian, consulting physicians, and our Medical Director.These specialists, along with our entire staff, work together to provide a place that feels like home.

Our on-site rehabilitation team includes physical, occupational, and speech therapists, and specially trained therapy assistants, all dedicated to helping you regain strength, mobility, and independence, and achieve the best possible outcome. Our experienced team of rehab professionals is here seven days a week, as ordered by your physician, with all of the equipment needed for effective rehabilitation.

We start planning for discharge upon admission, creating personalized treatment plans that implement physician's orders to meet personal goals. Our 24-hour skilled nursing teams provide the care and attention needed to maximize recovery, and a family meeting is held within 72 hours of admission to review expectations and address any concerns.
