If you are unable to print, or forget your forms at home, we will have them available to you in the office for your convenience. Please arrive 15 minutes prior to your appointment time to complete your paperwork and ensure you are seen as scheduled. Also, make sure that you bring your photo ID, as well as a list of any medications and xrays/records that you have.

Dr. Nadia Ramirez is originally from San Juan, Puerto Rico, and entered Life University's Doctor of Chiropractic program in Marietta, GA where she continually represented her class in the Dean's List and graduated cum laude. Dr. Ramirez has two little girls who have been under chiropractic care their entire lives.
