Bring your insurance card, photo ID, physical therapy prescription, diagnostic test reports, medication list, and wear comfortable clothing with sneakers.
What does a typical therapy session include?
Sessions include neuromuscular evaluation, functional training, postural rehabilitation, massage, joint mobilization, therapeutic exercises, and modalities like ultrasound or electrical stimulation.
What insurance plans do you accept?
We are contracted with Medicare and Blue Shield, and can bill most other insurance companies as an out-of-network provider.
What are your office hours?
Monday, Wednesday, Friday: 7:00 AM - 4:00 PM; Tuesday and Thursday: 10:00 AM - 7:00 PM. All treatments by appointment only.
