Cancellation policy: 24-business hour cancellation notice by telephone or text for follow-up visits. First office calls require a 48-business hour cancellation notice by telephone or text. Monday follow-up visits must be cancelled or rescheduled by the previous Friday; Monday first visits by the previous Thursday.

For your first office visit, please print out, complete, and bring in the hard copies of the 1. Health History, 2. Consent to Treat, and HIPAA forms. Your first visit will take approximately one hour and 15 minutes; follow-up visits are approximately 50 minutes. I will review your complete health history.
