The initial office was at Market and Wells Streets and the Lahaina office was located in the Pioneer Mill Plantation Hospital.

Contact your insurance carrier to inform them of your request to change PCP or Health Center and give reason for change. They will provide you with a list of participating physicians in their plan. Depending on type or severity of complaint, you should call the Wailuku office at (808) 242-6464 and direct your complaint to the following areas: Customer Service Supervisor, Nursing Director or Administration.
