Chronic care management, is the combination of supervision and education programs overseen by healthcare professionals to assist patients diagnosed with chronic diseases and health conditions such as Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), End Stage Renal Disease (ESRD) & Chronic Kidney Disease (CKD) and diabetes.

An individual may be referred to Alana Healthcare if their insurance company or other entity is contracted with Alana Healthcare. In addition, the individual being referred must have been diagnosed with or recently hospitalized with COPD, CHF, ESRD, or Diabetes. All referred individuals will receive a letter and a phone call providing more information about the program.
